Self & Match Considerations Guides
The Self & Match: Considerations PRIOR to Implementation Guide is your roadmap for setting up each individualized Self & Match System. We encourage you to collaborate with a team as you complete the form. Below you will find a downloadable Self & Match Considerations Guide that you can print or fill-in digitally as you plan your Self & Match Systems. You’ll also find a Self & Match: Considerations DURING Implementation Guide. This supplemental guide is a great tool to identify ways to enhance and improve Self & Match Systems that are already in place. We discuss the Considerations DURING Implementation Guide in more detail in our Advanced Workshops. You can learn more about these workshops here.
Creating Self & Match Forms
As described in the Self & Match manual, there are 5 easy ways to create Self & Match forms for students you support.
- Create a form from scratch by opening and creating a document.
- Use Self & Match Maker to create a customized downloadable form.
- Download a template from below and adjust the template to meet the needs of your student.
- Customize one of the digital Self & Match forms that you’ll find below.
- Request an editable sample of one of the forms in this manual.
For access to a specific sample form that is included in the manual, please email team@selfandmatch.com.
*In order to ensure the integrity of the program, Self & Match forms should be individually created for each student and should not be universally copied or distributed without systematically completing the Considerations Prior to Implementation Guide first. In addition please note you are not permitted to post these forms or others that you have created to any resource sharing websites such as Teachers-Pay-Teachers.
*When referencing the Self & Match name, please note that it is Self & Match and never “Self and Match”, “Self n Match”. “Self Match”, or “S&M.” The Self & Match logo may not be altered in any way. Additionally, all forms should also include (Salter & Croce, Year) on the bottom right footer in size 8 font.
